Points-based loyalty program
Submit CandidPro Comprehensive, CandidPro Elements, or PermaForm retainer cases to earn points during your loyalty qualifying period. Once you reach a higher tier's point threshold, your upgraded status is active for the rest of the current period and the entire following period.
While all tiers above Standard provide a discount on Comprehensive cases, please note that no discounts apply to Elements or PermaForm cases.
Loyalty qualifying period
Your loyalty qualifying period lasts six months, always beginning on the 15th and ending on the 14th.
To align with this schedule, your official "reset date" is rounded up to the nearest 15th after you sign up. For example:
Sign up May 8: Your first period officially starts May 15.
Sign up May 19: Your first period officially starts June 15.
Because you begin earning points the moment you sign up, your very first qualifying period will include those extra days or weeks as a bonus. After that initial period, your account will reset every six months on your designated date.

Transitioning to the New Points-Based Program
We are migrating accounts from our legacy case-based system to the new points-based loyalty program on April 15th, 2026. Below is everything you need to know about the transition.
Does my current loyalty status carry over?
Yes. To ensure you don’t lose any progress during our transition to the points-based program, we will honor whichever tier is higher:
Your existing legacy tier (Standard, Silver, Gold, Platinum, or Diamond); OR
The new tier you’ve earned by converting your cases from the current period into points.
This calculation includes all qualifying CandidPro Comprehensive (100 points), CandidPro Elements (50 points), and PermaForm retainer (10 points per set) cases submitted during your current loyalty qualifying period.
Is my reset date changing?
Yes. All loyalty qualifying periods will now run on a fixed calendar where they consistently end on the 14th of the month and restart on the 15th of the month.
To align your account, your current reset date will be "rounded up" to the nearest 15th of the month.
Important: Your period will not be shortened. This transition is designed to ensure your current qualifying period remains at least six months long; any extra days gained from "rounding up" are added as a bonus to your initial period.
Frequently asked questions
When does the tier status upgrade I earned take effect?
Points are added to your account when you place a case order. Once achieved, your new status remains active for the rest of your current qualifying period and the entire following period.
Note on PermaForm Retainers: While most points are instantaneous, please allow 1–2 business days for PermaForm retainer points to reflect in your account.
Do Airway-friendly cases earn loyalty points?
Yes, Airway-friendly cases are considered Comprehensive and earn 100 points per case.
Do PermaForm retainers earn 10 points per case or 10 points per set?
PermaForm retainers earn 10 points per set (one set includes two trays). This means if you order a package containing 3 sets of retainers, you will receive 30 points rather than 10.

Do individual doctors in a practice have different loyalty programs?
No. The loyalty program is applied at the account level. This means that if an account has multiple providers, all of their combined case submissions contribute to the same point total and tier qualification. The thresholds required to reach a higher tier are based on the total volume of every doctor on that account.
Because loyalty tiers are tied to accounts, a provider who works at multiple locations may see different case fees depending on the specific loyalty status of the accounts at each practice.
